This kind of happened to me on PS4 official servers today. I looted a skeleton key chest, then the chest disappeared. I noticed other chests were not spawning. You can do damage to the chests during combat and they will disappear. Make sure to pull the boss away from the chest during combat. Dont mind the red * marker. Thats just a boss/chest location like normal. Might update with colour code with boss types. This is an older version of the map though, when I got time I will update my guide with the new map and locations. You can use that map as a template for the bosses/chests as soon as I got it ready! Note that the mod was uploaded on May 22, shortly after the game launched as V1.0 . The Skeleton Keys were added later, and the mod hasn't been updated since May 22.
